The new Campaign Forgotten scenarios advertise mines.

I am so confused.

I did some play testing and I discovered that you can move across a water hex without hitting a mine with the first ship. But that doesn't mean the hex is safe. A trailing ship may go through the same hex and strike the mine.

Maybe there is a probability of hitting the mine with each ship?

Also, do the mines populate randomly or are they always in the same hexes to start the scenario?

There was some discussions during testing whether to refer to them as mines or torpedoes. Eventually John Ferry wanted them called Torpedoes in reference to Farragut's famous quote “Damn the torpedoes, Full speed ahead!”.
Seems I did not make the manual headings match, the Editor Manual calls them Torpedo/Mine while the User Manual refers to them as Torpedoes.
From User Manual
Naval Torpedoes
Torpedoes are used in some scenarios involving naval units. They are placed in
water hexes in the Scenario Editor and are not visible to either side in the game.
During play whenever any gunboat regardless of side enters a torpedo hex there is
a chance it hits the torpedo and is sunk. This is indicated by the sound of an
explosion, the text Torpedo Explodes – Ship Sunk. The text does not show in
replays for PBEM and Hotseat games but the explosion is heard.
Once the torpedo explodes and sinks a ship it is gone.

I will correct User heading in the next update to read

The question does remain though why one ship can move safely through a hex and the one behind it hit the mine. Is there a probability involved when a hex is mined?

That's what the manuals makes clear, "whenever any gunboat regardless of side enters a torpedo hex there is

Also, with regards to another questions you had. The mines do not randomly populate, however, there a several versions of the same scenario(s) that have "torpedoes" populated in different hexes. And I believe, that once detonated, the torpedo is vacated from that particular hex.
